مقدمه و هدف. اثر تمرینات ورزشی بر نارسایی‌های متابولیک از طریق دست‌کاری عامل رشد فیبروبلاستی-21 (FGF-21) متناقض است. بنابراین، هدف تحقیق تعیین اثر کمّی تمرینات ورزشی بر FGF-21 سرم بزرگ‌سالان دارای اختلالات متابولیک بود. روش‌شناسی. ابتدا جستجوی پایگاه‌های اطلاعاتی PubMed و Google Scholar برای استخراج مقالههای فارسی و انگلیسی منتشره شده تا اردیبهشت 1400، انجام شد و اندازه اثر (SMD) با مدل اثرات تصادفی محاسبه شد. یافته¬ها: 14 مطالعه شامل 19 مداخله(شامل تمرین هوازی(3=n)، تمرین مقاومتی(3=n)، تمرین تناوبی شدید(4=n) و تمرین ترکیبی(4=n)) دربردارنده اطلاعات 503 آزمودنی(دارای اضافه‌وزن یا چاقی و یا مبتلا به دیابت دو، سندرم متابولیک و کبد چرب)، واجد شرایط ورود به فراتحلیل شدند. اما به دلیل وجود ناهمگونی، درنهایت اثر کمی 16 مداخله به صورت کاهش FGF21 سرم پس از تمرین[001/0p=، (22/-0 الی 65/0CI: -) 44/0-SMD=] به دست آمد که با سن(10/0=P) و شاخص توده بدن(50/0=P) آزمودنی‌ها همبستگی نداشت. بحث و نتیجه‌گیری: تمرینات بدنی بی‌اعتنا به سن یا وضعیت وزنی، ابزار کارآمدی برای کاهش قابل‌ملاحظه  FGF21سرم بیماران متابولیک هستند که انتظار می‌رود به بهبود بیشتر اختلالات متابولیکی منجر شود. با این حال، هنوز هم نیاز به کارآزمایی‌های بالینی بیشتر با در نظر گرفتن نقش مؤلفه‌های تمرینی از جمله شدت و نوع تمرین در این زمینه باقی است.
