پویایی زمانی سایتوکاین‌های پیش‌التهابی و ضدالتهابی، همراه با نشانگر التهابی hs-CRP، متعاقب یک جلسه فعالیت تناوبی شدید هوازی با حجم بالا در دوندگان استقامتی

چکیده
مقدمه: فعالیت شدید ورزشی می‌تواند پاسخ‌های التهابی را به‌طور حاد تغییر داده و بر ریکاوری ورزشکاران اثر بگذارد. هدف پژوهش حاضر، بررسی پویایی زمانی پاسخ سایتوکاین‌های پیش‌التهابی و ضدالتهابی و نشانگر التهابی hs-CRP پس از یک جلسه تمرین تناوبی شدید با حجم بالا (HIIT) در دوندگان استقامتی بود.

روش: در این مطالعه نیمه‌تجربی با طرح اندازه‌گیری‌های مکرر، ۱۰ مرد ورزشکار استقامتی با میانگین سنی ۲۰/۴ سال شرکت کردند. آزمودنی‌ها حداقل سه سال سابقه تمرین و شرکت در رقابت‌های لیگ داشتند و VO₂max آنها بیش از ۵۰ میلی‌لیتر/کیلوگرم/دقیقه بود. پروتکل تمرینی شامل پنج تکرار دویدن ۱۰۰۰ متر با شدت ۹۰ درصد رکورد شخصی بود. نمونه‌های خون در سه مرحله پایه، بلافاصله پس از تمرین و ۱۲۰ دقیقه پس از فعالیت جمع‌آوری شد. غلظت IL-6، TNF-α، IL-10 و hs-CRP با روش الایزا اندازه‌گیری شد. داده‌ها با تحلیل واریانس با اندازه‌گیری مکرر و آزمون بونفرونی در سطح ۰/۰۵ تحلیل شدند.

نتایج: اجرای پروتکل HIIT موجب افزایش معنادار غلظت IL-6، TNF-α، IL-10 و hs-CRP شد (P<۰/۰۵). بیشترین تغییر IL-6 بلافاصله پس از تمرین و بیشترین پاسخ IL-10 در ۱۲۰ دقیقه پس از تمرین مشاهده شد. افزایش hs-CRP نیز در ۱۲۰ دقیقه پس از فعالیت برجسته‌تر بود.

نتیجه‌گیری: تمرین تناوبی شدید با حجم بالا موجب ایجاد پاسخ التهابی حاد و افزایش هم‌زمان IL-10 می‌شود که بیانگر فعال‌شدن هماهنگ پاسخ‌های پیش‌التهابی و ضدالتهابی در فرایند ریکاوری است. تفاوت زمان اوج نشانگرها نیز الگوهای زمانی متفاوت تنظیم پاسخ التهابی پس از تمرین را نشان می‌دهد. این یافته‌ها بر اهمیت زمان نمونه‌گیری در ارزیابی التهاب و مدیریت مناسب تمرین و ریکاوری دوندگان استقامتی تأکید دارد.

Temporal Dynamics of Pro- and Anti-Inflammatory Cytokines and the Inflammatory Marker hs-CRP Following a Single Session of High-Volume High-Intensity Interval Training in Endurance Runners

Introduction: High-intensity exercise can acutely alter inflammatory responses and affect athletes’ recovery. This study investigated the temporal dynamics of pro-inflammatory and anti-inflammatory cytokines and the inflammatory marker hs-CRP following a single session of high-volume high-intensity interval training (HIIT) in endurance runners.

Methods: In this semi-experimental repeated-measures study, 10 male endurance athletes (mean age: 20.4 years) participated. All participants had at least three years of training experience, a history of league competition, and a VO₂max >50 mL/kg/min. The exercise protocol consisted of five 1000-m running repetitions at 90% of each participant’s personal best performance. Blood samples were collected at baseline, immediately post-exercise, and 120 min post-exercise. Serum IL-6, TNF-α, IL-10, and hs-CRP were measured using enzyme-linked immunosorbent assay (ELISA). Data were analyzed using repeated-measures analysis of variance and the Bonferroni test at a significance level of 0.05.

Results: The HIIT protocol significantly increased IL-6, TNF-α, IL-10, and hs-CRP (P<0.05). IL-6 showed its greatest increase immediately post-exercise, whereas IL-10 peaked at 120 min post-exercise. The increase in hs-CRP was also more pronounced at 120 min.

Conclusion: High-volume HIIT induces an acute inflammatory response accompanied by increased IL-10, indicating simultaneous activation of pro-inflammatory and anti-inflammatory responses during recovery. The different peak times of the investigated markers suggest distinct temporal patterns of post-exercise inflammatory regulation. These findings emphasize the importance of sampling time in evaluating inflammatory responses and may help optimize training and recovery management in endurance runners and in planning appropriate recovery periods after demanding training sessions.
